These persons may contact the Catholic School Department for further clarification."

One of the objectives of St. Charles Borromeo School is to promote the physical well-being and fitness of its students to provide a safe and healthy school environment.
All students enrolled in St. Charles Borromeo School must have a physical examination report on file in the school office and a record card of immunizations for polio, measles, DPT, hepatitis B, mumps, and rubella*. Enrollment is not valid unless both of these are filed. (* From time to time it becomes necessary to add health/immunization requirements as required by the state. Parents/guardians will be notified of these changes in a timely manner.)
CHILD ABUSE REPORTING In accordance with diocesan policy and California State law, school faculty and staff are obligated under penalty of law to report the reasonable suspicion of any physical or emotional abuse. This would include emotional deprivation, physical neglect, inadequate supervision, or sexual abuse or exploitation. In this very serious and legally narrow area, the school will not contact parents/guardians in advance of making a report to legal authorities. The clear intent of the law, based on the seriousness of the crimes listed above, is to mandate that a report of reasonable suspicion of abuse be made. School faculty and staff will make such reports in the best interest of the child and do not, once reasonable suspicion is established, have any legal alternative except to make the report to the proper authorities for their investigation and review.
HEALTH EDUCATION Health education is provided through classroom instruction and special programs designed to meet the needs of the developmental stages of the students.
HEALTH SCREENING TESTS Students at St. Charles Borromeo School participate in the following screening tests during the year: Hearing, vision, scoliosis, scalp/hair checks (when necessary)
ILLNESS AND INJURY Parents/guardians will be contacted immediately for information and instructions in the event of injury. If the parents/guardians are not available, the Emergency Form shall be consulted.
If no responsible person can be reached, and the illness or injury is believed to be serious enough to require medical attention, the person in charge will arrange for the student to receive immediate treatment from a doctor, hospital, or emergency care personnel.
MEDICATION When it is necessary for a student to take medication during school hours, these state directives must be followed:
There must be a written statement from a physician detailing the method, amount, and schedules by which such medication is to be taken.
There must be a written statement from the parent/guardian indicating the desire that the school assist the pupil in the matters set forth in the physician's statement.
All medication must be left in the school office. Students are not allowed to have ANY medication (this includes aspirin and inhalers) on their person or in their desk, backpacks etc. Students are sometimes tempted to share medications without the knowledge or consent of authorized adults. Any medication may have an adverse effect, and in these cases it is extremely important to know precisely the type of medication ingested for proper and immediate treatment.

ATTENDANCE Regular attendance at school is compulsory.

If a student is absent, the parent/guardian must call the school office before 9:00 a.m. If this call is not made by the specified time, a person designated by the school office will place a call to the parent/guardian to ascertain the whereabouts of the student.

If a student reports to school after 10:30 a.m. the student is marked absent for a half day.

If a student leaves school before 10:30 a.m. and does not return that day, the student is marked absent for the entire day. If the student leaves school after 10:30 a.m. and does not return that day, the student is marked absent for a half day.

An absence of a half day causes a student to lose a perfect attendance record. Any student tardy more than four times during a school year forfeits his/her perfect attendance record.

A written excuse from a parent/guardian is necessary when a student is absent. This note should include date(s) absent or tardy, reasons for absence or tardiness, and parent/guardian signature. This note should accompany the student on the day of his/her return to class.

A student may NEVER leave school alone during school hours.

All students must be signed out in the office by a parent or designated adult when leaving school during regular school hours. Classroom teachers will be notified when students have been signed out.

It is the responsibility of the parent or guardian to ensure that his/her child(ren)are on time for school. The school day begins at 8:00 a.m. for grades K (a.m.) - 8 and 12:00 p.m. for K (p.m.). Children arriving after these times are considered tardy.

The tardy student is to report to the office with a reason for her/his tardiness. The office will give the student a Classroom Admission form.

The school discourages extended absences for vacations and other non- emergency trips. A student will be expected to make up all work and tests which the class completed during his/her absence upon his/her return. Parents should not expect teachers to have specific assignments prepared for the student prior to the scheduled absence.

Excessive absences and tardiness can adversely affect a student's progress. If this is the case, parents/guardians will be notified and a review of the student's progress will be made. Recommendations will then be given to the parent/guardian regarding the means to be taken to ensure that the student will remain current in his/her academic studies.

FORMS

Every family must fill out an Emergency Form. All information must be complete and correct. Please notify the office immediately of any changes in address, phone numbers (home/business), or emergency numbers on file. (It is also helpful to note any cellular or pager numbers.)

A California school immunization record form and a physical examination report must be on file before admission to St. Charles Borromeo School is considered final.

A form noting agreement and support of the school handbook and its policies is to be signed by parents/guardians and where appropriate, the student(s). A student is not considered enrolled unless this form is completed.

Registration forms may include the following:

Permission form for counseling

St. Charles Borromeo School engages the services of a professional counselor to assist students and their families. This permission slip gives the counselor access to students on a very limited basis. A parent may contact or request a meeting with the counselor.

Agreement noting the means of tuition payment Emergency form (see prior notation)

Registration form noting information concerning the student and his/her family.

Fingerprinting form to be completed by

ALL parents/guardians. Once a parent/guardian is fingerprinted it is not necessary to complete this procedure yearly. Fingerprinting must take place prior to the first day of school.

The St. Charles Borromeo School Administration shall take all precautions to ensure the safety of its students, both on school grounds and within the immediate vicinity of the school. While at school, and school-sponsored activities, the students are covered for accidents by a reputable insurance company. This program is mandated by the diocese.

Emergency procedures for extraordinary situations will be printed in the newsletter. In the event of an emergency that requires evacuation of students and personnel, Charles Mack School located directly behind St. Charles Borromeo School, will serve to house students and personnel. If evacuation becomes necessary, please contact Charles Mack School directly for information and necessary procedures.
